Where do I send my official FPGEC documentation?
The following official documents are required for all FPGEC candidates and must be sent to NABP:
- Unrestricted, permanent pharmacy licensing and/or registration documentation, depending on specific country requirements.
- FPGEC Attestation with 2 current identical photos (1 attached and 1 loose),
- Properly notarized photocopy of an acceptable current photo ID, preferably a passport, and
- Passing TOEFL iBT score report.
Licensure documents sent must be submitted in a signed, sealed, and stamped envelope from the issuing body.
Additional supporting documentation may also be requested. This includes marriage certificates, proof of name change, birth certificate, translations, or other documentation supporting the application. Review the Documentation Requirements section of the Bulletin for additional information.
The following documents must also be submitted to Educational Credential Evaluators (ECE), accompanies by the ECE Application and General Evaluation Report:
- Official pharmacy school transcript
- Official proof of degree in pharmacy
ECE may request additional supporting documentation. Read the Bulletin or visit the ECE website for details about the documentation required by ECE.
Any document, including all seals and stamps, not in English must be accompanied by an official word-for-word English translation that is prepared and certified by an official translator. Review the Documentation Requirements section of the Bulletin for additional information.
