Specialty Pharmacy Accreditation
The cost of Specialty Pharmacy Accreditation is dependent on the type of pharmacy seeking accreditation. Currently, Specialty Pharmacy Accreditation applicants fall into three categories:
Low volume specialty pharmacy – less than 20,000 specialty prescriptions dispensed per year
- Due at time of application:
- Application Fee: $4,500
- NABP Survey Fee: $8,500 per location
- Annual Participation Fee Year 1: $1,250 per location
- Annual Participation Fee Year 2: $1,250 per location
- Annual Participation Fee Year 3: $1,250 per location
Estimated total for three-year accreditation for a low volume specialty pharmacy starting at $16,750.
Medium volume specialty pharmacy – between 20,000-99,999 specialty prescriptions dispensed per year
- Due at time of application:
- Application Fee: $5,500
- NABP Survey Fee: $8,500 per location
- Annual Participation Fee Year 1: $2,250 per location
- Annual Participation Fee Year 2: $2,250 per location
- Annual Participation Fee Year 3: $2,250 per location
Estimated total for three-year accreditation for a medium volume specialty pharmacy starting at $20,750.
High volume specialty pharmacy – 100,000 or more specialty prescriptions dispensed per year
- Due at time of application:
- Application Fee: $6,500
- NABP Survey Fee: $9,500 per location
- Annual Participation Fee Year 1: $3,250 per location
- Annual Participation Fee Year 2: $3,250 per location
- Annual Participation Fee Year 3: $3,250 per location

- Accreditation program fees are subject to change.
- If your pharmacy is not successful at the survey, an additional survey and applicable fees will be required to continue with the accreditation process.
Additional fees may be assessed for the following occurrences:
- Time extension requests
- Protracted Policy and Procedure (P&P) reviews
- Delays related to survey scheduling
- Protracted survey remediation
- Ownership change processing
- Facility add processing
- Facility relocation processing
