- Due at time of Supply Chain Inspection Application:
- Eligibility Fee: $1,400
- Due at time of application:
- Application Fee: $3,280
- Annual Participation Fee Year 1: $440
- Annual Participation Fee Year 2: $440
- Annual Participation Fee Year 3: $440
Total for three-year accreditation is $6,000.
- Accreditation program fees are subject to change.
- If your facility is not successful at the survey, an additional survey and applicable fees will be required to continue with the accreditation process.
Additional fees may be assessed for the following occurrences:
- Time extension requests
- Protracted Policy and Procedure (P&P) reviews
- Protracted survey remediation
- Ownership and name change processing

- Expedited Eligibility Review Fee: $3,880
- Due at time of application:
- Expedited Application Fee: $11,800

Total for three-year accreditation is $17,000.
