During this webinar, participants will learn about the Institute for Safe Medication Practices’ (ISMP’s) Targeted Medication Safety Best Practices for Community Pharmacy, including the new best practices added for 2025-2026 and why these were selected for national action. The best practices are intended to inspire and potentially mobilize board members, board staff, and practitioners from around the United States in the adoption of specific consensus-based safe practices nationwide. These practices were selected to reduce or eliminate repetitive medication safety issues in community pharmacies that continue to cause harmful or fatal errors to patients.

Michael J. Gaunt, PharmD

Senior Manager, Error Reporting Programs, Institute for Safe Medication Practices
Michael Gaunt is the senior manager of error reporting programs at ISMP, which he joined in 2004. In this role, Dr Gaunt oversees the operation of ISMP’s three reporting programs and works collaboratively with the United States Food and Drug Administration to advocate for nomenclature, packaging, and labeling improvements. He participates in on-site risk assessments in community and specialty pharmacies to assess risks and identify opportunities to improve safety and reliability. Dr Gaunt also serves as the editor of the monthly ISMP Medication Safety Alert! Community/Ambulatory Care newsletter. He also works with the ECRI and the ISMP Patient Safety Organization (PSO) to conduct research and analysis of event data from various settings and to support PSO members as needed. Dr Gaunt received a bachelor of arts degree in history from the University of Notre Dame and a doctor of pharmacy degree with a certificate in drug safety from Temple University School of Pharmacy.
