Universal Nuclear USP <825> Inspection Form
USP General Chapter <825> was published on May 1, 2020, and became compendially applicable on November 1, 2023. NABP has revised the Universal Nuclear Pharmacy Inspection form to assist its member boards of pharmacy with conducting inspections of nuclear pharmacies. A separate module was created to evaluate adherence to USP <825>. Recognizing that these nuclear pharmacy practices are being performed within state-licensed pharmacies, the form also incorporates general pharmacy practice principles and requirements such as prescription processing and pharmacy security, among others. Additionally, NABP will utilize this universal inspection form when conducting Verified Pharmacy Program® (VPP®) inspections of nuclear pharmacy VPP participants.
The inspection form was created with input from nuclear pharmacists, NABP, and state inspectors, and addresses both sterile and nonsterile radiopharmaceutical handling. A PDF version of this form is attached to this memo. A Microsoft Excel version of this Nuclear Pharmacy Inspection form is available to the states with conditional formatting. States may reach out to obtain a copy of the Excel version.
